The drug can be taken by mouth, as capsules or liquid, or given by injection. It interrupts fungal growth by interfering with the organism’s attempts to build a cell membrane, the protective barrier that each fungal cell uses to wall itself off from its surroundings (imagine if something was preventing your body from maintaining your skin - you would have difficulty surviving). Sporanox is used for the treatment of various fungal infections, including tissue and lung infections, and onychomycosis (fungal infection of the fingernails or toenails). People who have problems with their immune systems, such as organ transplant recipients and AIDS patients, are more susceptible to fungal infection, and this powerful drug is useful for treating their diseases. When the problem is uncomplicated onychomycosis, however, the drug is only recommended for people with healthy, normal immune systems. Sporanox is used for fungal nail infections, but it is probably not the best, or the most popular choice. Studies indicate that it may only work about half of the time, and potential Sporanox side effects range from mild to extremely serious. Itraconazole should not be given to pregnant women, to anyone who has a history of congestive heart failure, or to anyone thought to be allergic to Sporanox or related drugs. Rarely, patients have suffered serious liver damage, and dangerous interactions with other drugs are also a concern. When Sporanox is used for serious life threatening fungal infections, the benefit may outweigh the risk; however, for a simple fungal nail infection, patients should weigh possible Sporanox side effects, and the probability of success, against the primarily cosmetic benefit. Is Sporanox worth it? Patients should talk with their doctors about the alternatives for treatment. There are other prescription drugs available (none come without side effects, however) that work better. There is also a host of natural remedies and traditional treatments which, while not clinically tested, are generally much cheaper and safer than the prescription drugs. When the problem is not serious, these consumer-tested remedies are worth a look. Most of them are topical remedies, so the risk of Sporanox side effects, or other problems related to an oral medication, are removed. When natural remedies are taken orally, they consist of everyday foods or herbal mixtures (herbs can be toxic too, so do your own research) unlikely to cause problems. By David Snape I live in the midwest, and frankly speaking there are a lot of overweight people here. I know this because I’m originally from the East Coast and, also frankly speaking, there are a lot more skinny people there. I have also noticed that the West Coast sports a lot of skinny people too. Please, don’t get me wrong, the East and West Coasts have plenty of overweight people as well. I’m just saying on average it seems that there are more skinny people on the two coasts. Who cares about that? Yes, it really and truly doesn’t matter what a person’s weight is. It doesn’t matter if they are overweight or underweight or somewhere in the middle. It really does not matter, not one bit. Unless, and this is key, unless the person himself is not happy with his weight. Sure, there are a lot of other concerns that go with excessive weight, such as a higher risk for heart disease, elevated cholesterol levels and more pressure on the heart to perform. The heart itself is a muscle. Consider that the more weight a person has the more work that muscle has to do. I’ve heard it said that every pound of fat adds a mile more of blood vessels to pump blood through. That is Dr. Judah Folkman’s estimate. Dr. Folkman is a Harvard Medical School professor. You have to love medicine for bringing us information. Also consider that the heart never stops, under normal circumstances. It could be considered the hardest working muscle in the body. The heart is absolutely amazing when you imagine that it is busy working, continuously pumping blood through the body for your entire life. I’d hate to think that I was giving my heart additional work to do every time I put on ten pounds of weight. I love my heart, it keeps me going. So I want to be kind to it. Life can be an enriching experience. So I want to cherish the health of my heart so that I can get the most out of life. Anyway, back to weight loss. I love my body, so I’m motivated to care for it. I don’t look at weight loss as a chore. I am a skinny guy, especially for the midwest. But, I do have times when I start to notice my belly expanding. That usually serves as a wakeup call for me. So, what do I do when that happens? I start to cut way back on the junk food. I go to the health food store on Saturdays and I load up on organic strawberries, blueberries, celery, carrots, apples, other organic fruits and vegetables and organic skim milk. Why do I choose organic? I believe, correctly or incorrectly, that organic foods are healthier and have less toxic poisons on them. I know that is up for debate, but I want to err on the side of caution. As I mentioned before, I love my body and want to care for it the same way it cares for me. I once met a woman that worked for the EPA, she told me that apples have the most pesticides on them out of any of the food they monitor. Now I heard that second hand folks, so take it with a grain of salt. It may or may not be truth. Oh, what do I do with all that organic stuff. Well, I whip out my high powered blender and start making fruit smoothies. I take sliced celery and carrots to work with me and snack on them all day long, instead of the junk food. I also drink as much water as possible. I add minerals to my water. Within a week, I have generally start to get rid of the extra pounds. I keep this up for as long as it takes. There is one notable exception. I pick one day a week, where I’m allowed to splurge away on whatever I want. I’m 5 foot 9 inches tall and weigh about 165 pounds. Not bad. Not bad at all. A variation in latitude and height above sea level as well as the effect of ocean currents causes a great variety of climatic conditions throughout the country. There are three main rainfall-regions, divided into a winter rainfall in the south western part of the country including the Western Cape, rain all year round along the southern coastal belt and a summer rainfall in the rest of the country. Rainfall varies between around 125 mm per year along the arid western coast and 1 000 mm a year on the eastern coast. Rainfall is irregular with less than 10% of the country receiving around 750 mm of rain per year. The other 90% receive less than this. The summers in South Africa are very hot with mostly cloudless days, but in the winter, only a few areas are frost-free. If you travel to South Africa during October to May (mainly) you can go to farms and pick your own fruit. Make sure you check what times they are open so you won’t be disappointed. During November you can watch out for Strawberries. December is the correct month for strawberries, raspberries, loganberries, boysenberries, cherries and peaches. In January you can pick black currents, apricots, cherries, raspberries and strawberries. February is the month for raspberries, strawberries, apricots, nectarines, peaches and apples. During the fall you can pick raspberries, strawberries, nectarines, peaches and apples and during April you can still pick some strawberries, apples and even grapes. So when can you expect to buy certain fruits? Dont be caught out, just take a look at which fruit is in season during the year. These include baby marrow, butternut squash, various types of pumpkin, green beans, various other beans, carrots, beetroot, broccoli, cauliflower, cassava, cabbage, cucumber, mushrooms, onions, potatoes, sweet potatoes, peas, spinach, sweet peppers, tomatoes, lettuce, mealies (white mealies and sweet corn, which is yellow) and many more. Potatoes are grown all over the country, so when one part has a winter rainfall and the other a summer rainfall, they transport the produce to the necessary area. Mealies are planted during November and harvested during the month of March. As this finishes, the sunflowers and wheat are planted. Make sure you know which fruits are in season so you know what to look forward to when you travel. By Sharon Kirby What is Body Mass Index? Body Mass Index (BMI) is a statistical measure of body fatness, involving a calculation based on weight and height. The range within which the BMI falls determines whether a person is of a suitable weight, which may otherwise lead to related health problems. The BMI is a broad measure, and is not without its critics, but it is a straight forward, inexpensive method that can be used by women and men aged 20 and over to assess their weight in relation to the general population. This is not suitable for pregnant and breastfeeding women. Also note that when interpreting BMI for children, age and gender need to be taken into account. In any case, BMI should not be used in isolation when assessing an individual’s health status. How to Calculate Body Mass Index BMI can be calculated using metric units (kilograms and metres) or imperial units (pounds and inches). The metric system is more widely used. Metric Calculation Weight in kilograms is divided by height in metres squared. Weight (kg) / height2 (m) Here is an example: Height = (1.58 m), Weight = 54 kg Calculation: 54 (1.58)2 = 21.63 Imperial Calculation Weight in pounds is divided by height in inches squared and multiplied by 703. Weight (lb) / height2 (in) x 703 Here is an example: Weight = 119 lbs, Height = 5′2″ (62″) Calculation: (119 (62)2) x 703 = 21.76 Interpretation of Body Mass Index The range within which a person’s BMI falls will help determine whether they are of a healthy weight for their height. The ranges are as follows: Starvation < 15 Underweight 40 Morbidly obese 40 > In the example given in the calculations above, the BMI value is between 21 and 22, which is normal for a person who weighs 54 kg at 1.58 m. This is an indicator of good health, and there is no need for a person who has a BMI within the normal range to lose or gain weight. A BMI of 18.5 or under may indicate someone is deliberately restricting their food intake (or adopting other strategies) in order to achieve a desired level of thinness. This is unhealthy, and may lead to health problems such as malnutrition and osteoporosis, which are associated with the eating disorders anorexia and bulimia. BMI is indeed one of the criteria stipulated by the World Health Organisation for a diagnosis of anorexia nervosa, whereby BMI must be 17.5 or less. In some cases the fashion industry is using BMI as an indicator of whether models are too thin for the catwalk. A person with a BMI of 25 or over is probably overweight, which means they are at a greater risk of heart disease, osteoporosis, diabetes, high blood pressure, stroke, some cancers and other diseases. Steps should be taken to lose weight, including a healthier diet and regular exercise. There is the need to create a calorie deficit (burning more calories than are being consumed). For someone with a BMI of 30 or over, advice from a health professional about how to lose weight may be beneficial. Limitations of Body Mass Index While BMI may be a useful tool in monitoring someones health status, it has its limitations. BMI measures total body weight, not the actual amount of fat a person is carrying. Some people are naturally stocky and have a BMI that is in the overweight category, when in fact their weight is due to muscle mass and a heavier bone structure rather than excess fat. It is common for athletes such as rugby players and weight lifters to have a BMI indicating they are overweight when they are not. Their BMI is higher because of their extra muscle mass, not because of excess body fat (muscle weighs more than fat). Conversely, some athletes will be underweight according to their BMI (such as long distance runners). This is due to low body fat and aerobic slow twitch muscle fibres, which develop naturally as a result of their particular sport. Some people are thin-framed and their BMI will inaccurately determine them as being underweight. Those who have lost muscle mass (such as the elderly and infirm) may also appear to be underweight, when in fact it is normal to lose muscle mass and body fat in old age and as a result of poor health. On certain occasions the way BMI is interpreted can be adjusted according to the person’s lifestyle, natural build and health status. If BMI falls within the normal range, this does not necessarily indicate someone is in perfect health, that they are getting enough exercise or eating a healthy range of foods. They may also be carrying more body fat than is good for them. In addition to body weight, shape is also important. A person’s BMI may be normal but their waist circumference may be too high.
